• Fitness for any alternative duties as specified by Manager in the referral letter

• Likelihood of attaining fitness for own job in timescale suggested by manager

• Adjustments to enable a return to work

• Whether permanently unfit for own job

• Whether condition is likely to be covered by the Equality Act
